How do I know if my photos are on someone else's phone and how do I prevent this from happening?

Why do you think your photos are on someone else's phone? Have you shared an album with them?
Use Shared Albums in Photos on your iPhone, iPad, and Mac - Apple Support
If you've shared an album with someone and you want to stop, remove them as a subscriber:
Remove a subscriber
- Open the shared album.
- Press the People button
.
- Select the subscriber you want to delete.
- Tap Remove Subscriber at the bottom of the screen.
As the creator of a shared album, you determine who has access to it. You can remove subscribers at any time.
Keep in mind that they may have photos from the shared album saved to their phone. The only way you'll know is if you ask them.

Go to Settings/[your name] and view the list of devices that share your Apple ID. They can all see your photos if Photos is turned on in Settings/[your name]/iCloud/Photos. Remove any devices you don't want to share with.
Then open the Photos app, tap Albums, then tap Shared Albums (if available). On each shared album and within the album, tap the little "people" icon at the top. It shows the people with whom the photos in that album have been shared. Again, you can delete anyone you no longer want to share with.
Photos may have ended up on someone else's phone if someone you shared them with forwarded them or if you use a cloud photo service (not iCloud) such as Google Photos and then shared with others who use that photo service.
